How to Lodge a COIDA Injury on Duty (IOD) Claim — The Correct Process

Under COIDA, both employee and compliant employer are protected — but only if the IOD is reported correctly and on time.

1️⃣ What Counts as an Injury on Duty?

Injury, illness or disability arising out of and in the course of employment (including work travel).

  • Falls, burns, cuts
  • Machinery or lifting injuries
  • Vehicle accidents on company business
  • Chemical/smoke exposure
  • Repetitive strain & occupational diseases

2️⃣ Employer Completes W.Cl.2 (First!)

The employer — not the doctor — starts the process. Complete the Employer’s Report of Accident (W.Cl.2) immediately and send it with the employee to the doctor/hospital.

3️⃣ Doctor Completes W.Cl.4

Doctor fills the First Medical Report (W.Cl.4) on the same day and gives copies to employer and Compensation Fund.

4️⃣ Lodge Claim Online (Within 7 Days)

Submit at: gov.za Compensation Fund portal

Upload: W.Cl.2 • W.Cl.4 • ID copy • Payslip

5️⃣ Ongoing Reports

W.Cl.5 (Progress/Final) • W.Cl.6 (Resumption of Duty)

6️⃣ Pay Employee 75% Salary (Up to 3 Months)

Compensation Fund later reimburses the 75% (the 25% shortfall is on the employer).

7️⃣ Reinstate Employee

When doctor issues W.Cl.6, employee returns to work and claim is closed.
